***If your child will be attending the Ninth Grade Center and you requested bus transportation, please call the Transportation Department for the pick-up time/location and drop-off times/locations. They can be reached at 830-627-6150.

The goal of accepting the student-athlete forms electronically is to conserve resources and allow NBISD to be more efficient in collecting these annually required documents. Any desktop computer, iPad, or smartphone can be used to complete and submit forms.

All forms are needed to update medical history and annual physicals. They are required to be on file with the athletic training department prior to participation in any practice, scrimmage, or contest before, during, or after school. Medical history and the annual physical exam will be the only "paper" forms accepted.  Scanned PDF of these forms from home or your phone are also acceptable. Both Apple and Android phones have this capability. 

PHYSICALS MUST BE DATED AFTER MAY 1ST TO BE CONSIDERED CURRENT FOR THE FOLLOWING SCHOOL YEAR.
 
COVID-19 Update: UIL Requires all student-athletes testing positive for COVID-19 to have written clearance from a physician stating it is safe for your child to return to athletics. Generic return-to-school notes cannot be accepted. You can get a copy of the NBISD form by contacting the athletic trainers.

Vaccine Requirements for Students Grade K-12

Texas Minimum State Vaccine Requirements for Students Grade K-12

A student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child-care facility or public or private elementary or secondary school in Texas.

Please provide your shot record to the School nurse if your child has received these vaccines.  Your student will not be able to receive a schedule or attend school until the shots are received and written record is provided. 

NBISD has licensed the Frontline Health Portal to provide you with convenient, 24-hour online access to items in your child’s health record.  Using a secure username and password, you can view district-designated health information, communicate with our health & wellness staff, review/sign forms, and even provide the district with new information from outside health services received by your child. The District has a vital interest in maintaining a safe and healthy environment for all students.  The use of illegal drugs poses serious health and safety risks to the user and those associated with the user.  The District intends drug testing to serve as a deterrent to the use of illegal drugs and to provide students with a viable reason to resist peer pressure to use illegal drugs. All students in grades 7 through 12 who participate in school-sponsored competitive extracurricular activities shall be required to undergo random drug testing. All students in grades 9 through 12 who receive a permit allowing them to park their vehicles on school property during the school day shall be required to undergo random drug testing.

For the benefit of parents who do not have insurance or have limited health insurance, the school district is making available a Voluntary Student/Athletic Accident Insurance Plan.

The after-school program is operated by the YMCA of San Antonio and available for all elementary students at select school sites.
